Permission to use Photograph/Video

As part of our communication activity, Lions Eye Health Program- Australia (LEHP) use photographs and video for publicity and educational purposes. LEHP seeks your permission to use a photograph and/or video of you or your child for possible inclusion in LEHP and \/or Lions Australia publications such as newsletters, brochures, posters magazine articles, presentations and web content.
